This form should only be completed if we have contacted you to advise that your fostering application has progressed to stage one of the assessment process.
As part of this statutory process, we need to carry out a number of statutory checks (see below).
DBS check
We are required by law (Fostering Regulations 2011) to undertake an enhanced Disclosure and Barring Service (DBS) check in relation to prospective foster carers and each member of the household aged 18 or over. For DBS purposes, each applicant requires their own email address.
- A check will also be undertaken with the Local Authority where you live or have lived within the past 10 years.
- Details relating to finance - including household income and expenditure will also be sought.
- Confirmation will be sought from your current employer of your employment status.
- Social Media checks will be undertaken within the assessment process.
Reference checks
Other references will be routinely sought in the following circumstances:
- Where you are currently or were previously employed either in a paid or voluntary capacity or involving leisure pursuits with children or vulnerable adults, a reference will be requested.
- Where you have school age children, their schools will be contacted.
- Where you have had previous significant relationships, former partners will be contacted.
- Where you have served in the armed forces, SSAFA will be contacted.
- Where you have children living elsewhere, they will be contacted.
- Your doctor will be contacted and asked to conduct a medical examination to assess whether there are any medical issues which may affect your application, or your suitability to become a foster carer
Please complete this form as soon as possible as we cannot proceed with the assessment process without it.
