Please use this form to let us know about a parent who has withdrawn their child from your school to be educated at home.
Before you complete this form make sure you have a copy to attach of:
- Parent’s letter informing you of child’s home education
- the child’s most recent student attendance marks
If you think that there is a safeguarding risk to the child please refer to the Right Help, Right Time A continuum of need.
By completing this form, you are also meeting your statutory duty to supply us with information on children who leave school outside the standard transition points.
