This form provides a means for Children's Services to refer young people coming into adulthood to adult social care.
This form should be completed by the young person's allocated children's social worker or children's team representative.
In this form you may be required to upload the child's:
- EHC plan
- Pathway plan or needs assessment
- Capacity assessment
You will need to have details of the young person's GP before you should complete this form.
If the young person has been diagnosed with any medical conditions (evidenced through clinical assessment) evidence will need to be uploaded.
