Please complete this form if you wish to apply for a volunteer or professional chaperone licence.
To apply for a chaperone licence you must be over 18.
Applications will only be accepted for people that are residents of West Northamptonshire.
Please be aware that Chaperone licences can take up to 3 months to be issued.
Before you start:
- Please ensure you have a colour head and shoulders photo of yourself against a plain background, do not include other people in the photo, which you will need to attach to this application.
- Please ensure that you have the details of two responsible persons who would be prepared to give you references as to your suitability to be a chaperone. References cannot be accepted from a spouse, partner or family relation or from someone with whom you live.
