Finding the right support if you're at risk of a fall

Falls are not an inevitable part of growing older. There are things you can do to reduce the risk of falling. They include:

  • Speaking with your pharmacist or GP about the medication you are taking and to have a regular medication review.
  • Ensuring you have your vision and hearing checked regularly.
  • Making some changes to your home environment.
  • Doing exercises to improve strength and balance.

This form will ask you a series of questions. The answers will be used to determine the right support for you.

It is important to answer the questions honestly as there are several possible outcomes. This ranges from a referral to Falls Management Service, Supporting Independence Programme, Northamptonshire Sport, Wheelchair Services, and Drug and Alcohol Services. Our partner agencies all work together, and the assessment will enable us to refer you to the correct service.
