This form should be used to alert the Local Authority (West Northamptonshire Council) where a patient has been subject to a ‘prolonged period’ of seclusion (over 72 hours) or where a patient is in long-term segregation (over 72 hours). Definitions of seclusion and long-term segregation (LTS) have been taken from the Mental Health Code of Practice 2015.
