This form should be completed by any mainstream primary school setting who wish to refer a child to the Social, Emotional and Mental Health (SEMH) panel.
Please ensure you have read the information on the SEMH webpage to ensure you meet the criteria for referral before completing this form.
The deadline for submitting a referral form is 2 weeks prior to the date of the SEMH Panel meeting. You can find upcoming panel dates on the SEMH web page.
Please ensure that you demonstrate how you have implemented any support using the assess / plan / do / review cycle.
