This form should be completed by a school’s Designated Safeguarding Lead when you have received a domestic abuse notification about a child or young person:
- who attends your school and
- who has witnessed or been subject to domestic abuse.
This will inform West Northamptonshire Council of the type of support (silent or overt) that was offered to the child or young person as a result of you receiving this notification.
