To complete this form you will need:
- Proof of address dated within the last 12 months, which should either be a copy of a utility bill, or an official letter in your own name e.g bank statement, bill, council tax, Pension letter, or Letter from DWP.
- To be able to upload one of these documents during the application process.
- A police incident number if your blue badge has been stolen (you can obtain this by calling the Police on 101).
- Your debit or credit card to pay £10.
