Introduction
The Wellbeing Service works with customers to identify and address factors contributing to their issues and concerns, to support them in living independently in their own homes for as long as possible. The service is tailored to prevent decline, and to support and manage the health and wellbeing needs of anyone eligible over the age of 18, who may be showing signs of reduced independence.
With up to 12 weeks of one-to-one support, our team of dedicated Wellbeing Advisors are committed to providing you with the knowledge and resources needed to make your wellbeing a priority.
This could be supporting them to increase their levels of physical activity, helping them access or providing emotional or nutritional support and advice.
If eligible for the service, you'll be offered support with one of our Wellbeing Advisors over a course of 12 weeks.
Please note:
- We do have some eligibility criteria which will be assessed within this form.
- A referral means that the individual will be offered an assessment based on the responses to the questions in the form and where appropriate we will provide advice and support to address their needs. It does not guarantee a place on any specific programme.
