Once this form has been submitted, the contact person will receive a PDF copy of the application for their records.
This form is to be used to apply for a Small Society Lottery Registration. A fee of £40.00 will be collected at the end of this form and then the society will be invoiced for the £20.00 annual fee every year thereafter. The first annual fee is due 10 months after the grant of the registration.

Notes for applicants
This application will be refused if in the period of five years ending with the date of the application:
- an operating licence held by the society has been revoked under section 119(1) of the Gambling Act 2005 or
- an application for an operating licence made by the society has been refused.
The application may be refused if the Local Authority thinks that:
- the society is not a non-commercial society
- a person who will or may be connected with the promotion of the lottery has been convicted of a relevant offence or
- information provided in or with the application is false or misleading.
Once a registration has been issued and the Society has started operating, returns will need to be submitted every three months. To submit a return please download the required form and upload through our online platform. You will receive further information about this with your registration certificate.
