This application is to apply for a variation to the designated premises supervisor. This form will need to be accompanied by a DPS consent form.
You will need to:
Ensure the proposed DPS completes and submits the Designated Premises Supervisor Consent form, your application will not be complete, and consultation will not start until we have this. There are two ways to do this:
- You can download the consent form in a word format and attach it to this form, or;
- You can complete our online consent form

- An applicant’s agent (for example solicitor) may sign the form on their behalf provided that they have actual authority to do so.
