If you have a hackney carriage or private hire driver, vehicle or operator licence you should use this form to notify us if you have changed your name, address or if you require a replacement licence, badge or plate e.g. if it has been lost or stolen.
Please note for private hire operators you can only change the address of a licence holder, not the operator base address.
If you are changing your name or address you will need to supply us with the following:
- proof of name change e.g. marriage certificate, deed poll, or degree absolute (name change only, unable to process without this)
- amended visa, where applicable (name change only)
- amended passport, if you have one (name change only)
- amended driving licence
- amended vehicle insurance
- amended V5 Log book
With the exception of the proof of name change, these documents can either be uploaded to this form or alternatively the relevant suspension notice will be issued to request that these are supplied within 21 days of submission.
