You can use this form to give parent carer consent for West Northamptonshire Council Educational Psychologist support for children who attend schools in our area.
We need signatures for this form - so you will need to print and get it signed before returning it to us:
- if the child is 16 years old or over, they will need to sign
- the parent's signature (but if the child is 18 years old or over this isn't usually required)
